Announcement: Changes for Proxy of Teens Ages 12-17
What happens to my access if I already have limited access to my teen’s MyChart account?
Starting on February 12, 2025, if you currently have limited access to your 12-17-year-old’s MyChart account, your access will become more restricted in compliance with updated privacy regulations. Access to certain sensitive information, such as the ability to message or schedule on your teen’s behalf, will no longer be allowed.
What happens when my child turns 13?
When your child turns 13, proxy access to their MyChart account transitions to more limited access in compliance with privacy laws. This means proxies will no longer have access to certain sensitive features, like messaging or scheduling, unless explicitly granted by the teen. This ensures teens have control over their sensitive health data while allowing parents to maintain access to general information, such as allergies.
How do I request more access to my teen’s MyChart account?
If you need additional access to your teen’s health information, such as the ability to message a provider or schedule an appointment on behalf of your teen, you will need to:, the process is the same as with 12-year-olds turning 13. You can:
- Ask your teen to grant full access to their MyChart account.
- Contact our Support Team at 1-855-274-2517 or email at: mychart-support@intermountainhealth.org to discuss your situation. In some cases, such as dealing with ongoing medical conditions, the Release of Information Team may need to review and approve additional access based on your needs and your teen’s consent.
Why is my access becoming more limited?
This change is required to comply with state and federal privacy laws that protect the health information of teens ages 13-17. These laws ensure that teens have control over their sensitive health data while allowing parents to maintain access to general health information.
Can I request full access if my teen has a medical condition?
In cases where ongoing medical conditions or special circumstances apply, you can contact our Support Team at 1-855-274-2517 or email at: mychart-support@intermountainhealth.org to discuss your need for full access. The Release of Information Team may need to review and approve additional access based on your needs and your teen’s consent.

You can manage your family's health information through MyChart by requesting proxy access.

*Patient must have had an appointment with their provider in the past 400 days beginning May 1, 2023, or have an upcoming appointment in the next 180 days to message their provider in MyChart. Phone conversations are not considered appointments.
MyChart to MyChart Proxy Access – ADULT or MINOR Ages 13-17
A patient with an active MyChart account can grant proxy access to a caregiver/support person's MyChart account.
- The patient granting access must login to their MyChart account on the mobile app or on a computer at njhealth.org/mychart.
- Under Menu, select Sharing Hub.
- Answer, “Who do you want to share your health information with?” by selecting “a family member, close friend or caretaker.”
- Answer, “How do you want to share your health information with a family member, close friend or caretaker? By selecting “Give ongoing access to your MyChart account.”
- Select “Invite someone.”
- Fill out the fields to send someone an invitation to receive proxy access to your account.
- The person receiving access will be sent an email invitation. They will need to login to MyChart and accept the invitation as a proxy.
MyChart Proxy Request – ADULT to MINOR Age 17 and Younger
- Complete the Request for MyChart Proxy Access form.
- Submit completed MyChart Proxy Access form and copies of required supporting legal documentation (see list below) to the Health Information Management (Medical Records) Department on the main campus of National Jewish Health. You may deliver the form and supporting documentation in person, via fax or via postal mail.
Legal Documentation (Copies only)
If you are biological or adoptive parents please provide:
- Legible copy of Birth Certificate (reflecting parent’s name)
OR
- Patient Date of Birth and any two of the following relating to the patient:
Social Security Number
Account Number
Date of Service
Street Address
Insurance Carrier Name
Insurance Policy Number
Medical Record Number
Copy of Insurance Card
If you are a Legal Representative (foster parents, case worker, etc.) please provide:
- Copy of court order for guardianship
MyChart Proxy Request – ADULT to ADULT
- Complete the Request for MyChart Proxy Access form.
- Submit completed MyChart Proxy Access form and copies of required supporting legal documentation (see list below) to the Health Information Management (Medical Records) Department on the main campus of National Jewish Health. You may deliver the form and supporting documentation in person, via fax or via postal mail.
Legal Documentation (Copies only)
If you are a Legal Representative for the adult patient, please provide one of the following:
- Copy of court order establishing guardianship
OR
- Copy of power of attorney granting medical decision-making authority
Response Time
You can expect a response to your request for MyChart proxy access within 2 weeks of receipt by the Health Information Management Department.
